What are the patient criteria for the Vital Care program?
Any senior or commercial patient who is at risk for multiple admissions/emergency room visits, specifically:
Two chronic medical conditions + one high risk criteria (poor social support, functional status, psychological condition, substance abuse, and multiple admissions)
One poorly-controlled chronic medical condition + two high risk criteria
WHEN IN DOUBT, refer your patient (submit an authorization)
