How do I get an Authorization Tracking Number?
Your provider's office should be able to have either provided you with a printed document that displays your Authorization Tracking Number, or may have provided this to you by phone. This number can be used to enter on our website to receive the authorization status.

What if I was not provided with a printout at the provider's office?
You may call your primary care physician, referring specialist office, or our Customer Service Department at (818)357-5000, Monday - Friday from 8:00 a.m. - 5:00 p.m., TTY Dial 711, in order to ask for your Authorization Tracking Number.

What does my Authorization Tracking Number look like?
Example: 2018-0421-P010-0111
